Under Section 13402(e)(4) of the HITECH Act, the U.S. Department of Health and Human Services (HHS) is required to publicly post a list of breaches involving unsecured protected health information (PHI) affecting 500 or more individuals. These incidents are reported to the Office for Civil Rights (OCR) and include both resolved and currently under investigation cases.
This dashboard visualizes breach data from the HHS Breach Portal over the past 24 months, offering healthcare organizations, IT professionals, and compliance teams a clear view of current risks, affected entities, breach types, and potential impact areas.
